Primary Responsibilities: 

As part of a practice transformation team, this individual will lead statewide operations in the North Carolina market and contribute to the development of our national Population Health and Clinical Transformation Programs. We are looking for a critical thinking, dynamic, high-energy individual who loves to experiment with new ideas and build business capabilities for those ideas that prove successful Positions in this function drive clinical relationships and engagement with physician practices, clinically integrated networks,  members, and pharmacies while partnering internally ( Health Care Economics and Analytics, Medical Directors, Reporting, Health Plan market leaders) with a goal of improving health, well-being, quality and practice performance while reducing medical costs. Positions are accountable for the full range of clinical practice performance which may include but is not limited to improvement on HEDIS gap closure, coding accuracy, facilitating effective education and reporting, effective member engagement (e.g., members with complex and/or chronic conditions), and proactively identifying performance improvement opportunities through the use of data analytics, technology, workflow changes and clinical support. These roles develop comprehensive, provider-specific plans to increase physician practice performance, reduce readmissions and improve population health outcomes:

  • Serve as accountable owner for all quality and regulatory reporting requirements for Advanced Medical Home outcomes
  • Primary liaison for NC Healthcare Association
  • Leadership of ongoing operations, including direct management of a team Clinical Practice Consultants (CPCs) and Clinical Practice Performance Analysts (currently 8 staff)
  • Optimization of strategic program operations by developing processes for operational, data and clinical use and approaches
  • Develop and execute policies, procedures and workflows in support of the advanced medical home program as designed by NC and according to requirements set forth in the NC contract / RFP
  • Coordinate development of infrastructure around people, process, and technology to achieve scale, working in a matrix environment
  • Design and develop clinical and analytical capabilities that will enable the CPC Team, working closely with Medical Directors and clinical resources as well as with key stakeholders in Network (UHN), Healthcare Economics (HCE), or State / Regional / National Market leaders
  • Attend all Community and State plan meetings and other market meetings as appropriate and necessary
  • Drive execution of clinical practice transformation expansion plans by overseeing timely and successful deployment through the Clinical Integrated Networks who will work with practices and ongoing practice engagement, working closely with clinical front office staff
  • Lead a team of cross-functional members with operations and business analysis capabilities included charts reviews
  • Anticipates customer needs and proactively develops solutions to meet them
  • Serves as a key resource on complex and / or critical issues
  • Solves complex problems and develops innovative solutions
  • Performs complex conceptual analyses
  • Forecasts and plans resource requirements
  • May lead functional projects
  • Motivates and inspires other team members

 

  • Bachelor’s degree or equivalent

  • To be eligible to apply for this position, you must have had leadership experience in one of the following areas: Population Health (PH), Accountable Care Organizations (ACO) or Patient Centered Medical Homes (PCMH)

  • 8+ years of work experience which includes experience in cross-functional business areas such as general management, strategic planning, business development, and people management

  • 3+ years of leadership experience with direct reports

  • 5+ years of experience in the health care industry, preferably managed care (Medicaid)

  • Direct experience in population health and clinical transformation

Preferred Qualifications:

  • Strong influencer - this position will partner with Clinical resources to enable practice transformation

  • Strong relationships-this role will work with Clinical Consulting Directors, Medical Directors, Sr. Leadership as well as business and technology partners to plan, direct and develop clinical programs

  • Strong Communicator-this role will need to be extremely comfortable presenting program objectives, scope, and process across segments

  • Works effectively in an ambiguous environment

  • High degree of emotional intelligence

  • Direct management of remote/telecommuter or regional virtual team
